The provisional findings of an inquiry into the collision between Mewar Express and Goa Express has revealed that the accident occurred due to the failure of Railway Staff.<br/>The rear-end collision of the trains occurred on Mathura-Palwal Section of Agra Division of North Central Railway on the 21st of last month. As a result, twenty-two passengers had died, 16 were grievously injured and six sustained simple injuries in the accident.<br/>According to a Railway Ministry release,Mr PK Bajpai, Commissioner of Railway Safety, NE circle, Lucknow, Ministry of Civil Aviation , held a statutory inquiry into the cause of the accident. The findings of the Commissioner are under consideration of the Government.<br/>
