The derailment of Janata Express and Kamayani Express on 4th August this year in Harda district of Madhya Pradesh was caused due to washing away of ballast and formation of soil beneath the track on both up and down lines on the approaches of Railway Bridge. In provisional findings, Commissioner of Railway Safety, South Central Circle, Dinesh Kumar Singh said that the incident occurred due to failure of equipment and structure. He said that due to overflow of flood water, the track ballast washed away. <br/><br/>Mr. Singh, who conducted the inquiry into the incident, has recommended that Railway should consider installing water level monitoring system on the railway bridges with facility to generate warning and alerts in case the water reaches danger level. He has also said that the site of accident should be declared vulnerable location till adequate waterway is provided after proper review. Twenty passengers were killed and 45 people were injured in the twin derailment.
